4. These ‘master’ proteins protect us from deadly mutations — and could inspire new drugs(feature)

7. Vaccination generates broadly cross-neutralizing antibodies to the HIV Env apex(research;OA)
這篇 HIV vaccine 研究值得放進必讀,因為它不是單純提升 antibody titre,而是把 immunogen display、non-human primate response 與 cryo-EM structural validation 串起來。研究以 heterologous HIV Env trimers covalently coupled to liposomes 作 multivalent display,使所有 trimer-liposome-immunized non-human primates 出現 cross-neutralizing serum antibody responses,並從多隻 macaques 分離出能 cross-neutralize divergent HIV clinical isolates 的 monoclonal antibodies。結構上,這些 antibodies target Env trimer apex,方式類似人類感染後產生的 apex-directed broadly neutralizing antibody PG9。臨床轉譯上,這是 HIV vaccine 設計的實質進展,但仍是動物與免疫機制層級;還不能推論人體保護效果、持久性、boosting strategy 或免疫優勢路徑已解決。

8. Netrin1 blockade alleviates resistance to chemotherapy in pancreatic cancer(research;OA)
這是本期最接近臨床實作的研究。文章報告 phase 1b:first-line locally advanced pancreatic cancer 患者 43 人接受 NP137 加 mFOLFIRINOX every other week、最多 12 cycles。NP137 tolerability 良好;median PFS 為 10.85 個月,95% CI=10.03–15.61;median OS 為 16.43 個月,95% CI=12.75–non-reached;conversion surgery 發生於 23%。機制面,microbulk RNA sequencing 指向 EMT 下調,且 neogenin-high tumour 的 median PFS 為 15.65 個月,neogenin-low 為 10.22 個月。臨床讀法應保守:這是 early-phase combination trial,支持 NP137 作為 chemo-resistance/EMT axis 的候選策略,尤其可能需要 neogenin biomarker 分層;但尚未取代標準 mFOLFIRINOX,也不能用非隨機結果直接推論 survival benefit。
常見誤判:把 phase 1b 單臂或早期組合試驗的 PFS/OS 訊號解讀成已證實優於 mFOLFIRINOX。
